Applications are invited in the enclosed format for filling up of 01 post (Open Category) Gr.II in the pay scale of Rs 5200-20200 with grade pay of Rs 2800/- possessing qualification of Matric with Diploma in Library Science (2 years Course after Matriculation). Maximum age limit of 23 years as on 30.11.09. Relaxation upto 35 years to Government Servants and employees of the ESI Corporation. General Conditions: A Candidate should submit only one application. Submission of more than one application may lead to rejection of all applications. More submission of application does not confer any right on the applicant to be called for written test / interview. Application should be submitted strictly as per given format only and all columns should be filled in Block Capitals letters in the candidates own handwriting. Incomplete / Unsigned / Application record without photo/Certified copies of required documents or received after the last date will summarily be rejected. Wrong declarations / false information shall lead to cancellation of the candidature at any stage. No interim correspondence will be entertained. Canvassing in any from will be a disqualification and shall lead to cancellation of candidature.

Candidates may submit application in an envelope of the size 25x10 cm superscribed "Application for the post of Librarian Gr.II' and accompanied by attested copies of testimonials certifying the particulars of the applicant to the Jr Director, ESIC Model Hospital Cum ODC, Andheri(E), Mumbai 400093 on or before 15 Jan 2010 upto 4:00 PM. Applications should accompany a demand draft of rs 100/- drawn in favour of ESI Fund A/C No IESIC Model Hospital, Andheri (E) after the date of advertisement. Two copies of recent passport size photograph duly attested by a Gazetted Officer, one pasted on the application) and a self addressed and stamped (Rs 10/-) envelope of the size 10cm x 22cm PH/Ex Servicemen Categories and employees of ESI Corporation are exempted from payment of fees. Fee once paid will not be refunded. Fee in any other form will not be accepted. The candidate must write his name and address on the bank of DD. Incomplete application will be summarily rejected. Medical Superintendent may at his discretion increase or reduce the number of posts. No TA/DA will be paid to candidate called for written test / interview including reserved categories. Selection of candidates will be done on the basis of a written examination and / or interview.
